OK… here we go. Please fill out this form once for the whole “act.” Fill it out regardless of whether you are a solo performing artist or a performing arts ensemble.

1. What is the exact name of "act" that will perform? (Use only standard English characters. For a solo artist, please use the stage name)

2. How many members in group? (for a solo artist put "1")

3. How many support staff (engineer, driver, tour manager, or if this is a solo artist, how many people combined in backing group and crew?) will need a visa as well?

4. Who should be our primary contact for the artist? This could be the artist’s manager, or any other person in the artist's team who will work with CoveyLaw in handling the case.

10. Has the act ever won or been nominated for a major award (e.g., Grammy, Juno, AFRIMA, Mercury Prize, Olivier, etc.)?

11. What is the EXACT date of arrival in the U.S. for the artist? If unsure, put the earliest possible arrival date. THIS IS IMPORTANT: If you will not be available for a consular interview in the two weeks prior your arrival in the U.S., please tell us!!!

12. What is the intended visa end date?

NOTE: O visas for individuals can be valid for up to three years and P visas for groups and culturally unique artists can be valid for up to one year, but only if you can prove you have a contractual need for the visa to last that long—these long durations are never automatic! If you want a visa that covers more than the artist's immediate upcoming dates, let us know so we can discuss what it will take to make that happen. 13. At which U.S. embassy or consulate will the applicants complete their visa interviews (city and country)?

NOTE: As of September 6, 2025, the U.S. Department of State is instructing applicants to complete their interview in their country of nationality or residence.
